Finding the right support should not require you to become an expert in Oregon's I/DD system. Empowered Services helps people across Oregon understand approved services, start intake, bring someone they trust, or ask us to help find a DSP.

Your care team stays connected after services begin—supporting you, your caregiver, and your Services Coordinator or Personal Agent as needs change.

Empowered Services supports everyday life at home, at work, and in the community. We offer Attendant Care, Relief Care, Behavior Support Plan Implementation, and On-the-Job Attendant Care. Adults can also use Day Support Activities and Community Transportation.

ISP · Your plan

Your Individual Support Plan (ISP) is your plan. It says what matters to you, your goals, and the help you need.

ONA · Your support needs

If you need an Oregon Needs Assessment (ONA), it helps show how much support you need and any health or safety risks. Not everyone needs an ONA.

Your Services Coordinator or Personal Agent can help you understand both. If you are not sure what documents you have, that is okay. You can still contact us.

Attendant Care

Get help with personal care, meals, home routines, getting around, safety, and other daily needs in your ISP. If you have an ONA, it may also help show how much support you need.

Build skills, relationships, and community connections

Day Support Activities

Adults can get support to build or keep skills, relationships, and community connections. Your goals, choices, and ISP help shape where and how support happens—on your own, with a group, in the community, or in a facility.

Support when your usual caregiver needs a break

Relief Care

Your caregiver may need time to rest or handle other responsibilities. Relief Care gives you support during that time. Your ISP shows the Relief Care included for you.

Consistent support that respects you

Behavior Support Plan Implementation

If you have an approved Behavior Support Plan, trained DSPs can follow it during your support. The plan helps staff understand what helps you feel safe, communicate, and take part in daily life. We follow the plan with dignity and respect. We do not create it or provide independent behavior-professional services.

Support with your personal needs while you work

On-the-Job Attendant Care

Get help with personal care, communication, moving safely, health routines, and other needs in your ISP while you work. This is attendant care at work. It is not job coaching.

Community Transportation

For adults, Community Transportation may help you get to activities, work, appointments, and other places tied to your goals. Your ISP and Oregon transportation rules guide what is available.

Attendant Care

Help with daily life—at home and in the community.

Attendant Care can help with everyday activities included in your ISP. The examples below show common areas of support. Your ISP identifies the help planned for you, and if you have an ONA, it can help describe the level of support you need.

Personal care and daily routines

Respectful help with personal care, hygiene, reminders, household routines, and everyday tasks that protect dignity and keep daily life on track.

Community access and participation

Support with community activities, recreation, errands, volunteering, events, and other parts of everyday community life included in your ISP.

Meals and grocery routines

Support with grocery routines, food choices, meal planning, and kitchen structure when those activities are included in your ISP.

Meal preparation

Hands-on help with safe cooking and meal preparation when included in your ISP.

Everyday skills and routines

Coaching, prompts, and practice may support shopping, organization, budgeting, meals, communication, and other daily routines when those needs are included in your ISP.

Mobility and everyday activity

Help getting around safely and taking part in walking, recreation, and active routines at your pace and preference.

These are examples, not an authorization. Your ISP and your Services Coordinator or Personal Agent identify the services and supports approved for you. If you have an ONA, it can help describe your support needs.

New to Oregon's I/DD system?

You do not have to figure it out alone.

Eligibility and service authorization come through Oregon’s I/DD system. Your Services Coordinator or Personal Agent helps with your ISP and, if you need one, your ONA. Empowered Services does not decide eligibility or authorize service hours. You can contact us even if you are not sure what documents you have—we will explain what we need and what happens next with us.

A family member, friend, or longtime supporter may apply through Empowered Choice. Required background checks, onboarding, training, and employment steps still apply.

What services does Empowered Services offer?
Empowered Services provides statewide disability support for people with intellectual and developmental disabilities in Oregon. Services may include Attendant Care, Relief Care, Behavior Support Plan Implementation, and On-the-Job Attendant Care. Adults may also receive Day Support Activities and Community Transportation. Daily living skills may be part of your goals and everyday support rather than a separate service. Your Individual Support Plan (ISP) identifies the services and supports planned for you.
Who do you support?
Empowered Services welcomes service inquiries for adults and children with intellectual and developmental disabilities across Oregon. What support is available depends on your age, ISP, authorized services, goals, support needs, and qualified staffing. Empowered Services offers Day Support Activities and Community Transportation to adults.
Does Empowered Services work with the Children’s Extraordinary Needs (CEN) Program?
Yes. Empowered Services can help families participating in Oregon’s Children’s Extraordinary Needs (CEN) Program understand our parent-caregiver hiring and onboarding process. CEN eligibility and service decisions remain with Oregon ODDS and the child’s Services Coordinator. Parent or guardian caregivers must complete the required employment, background-check, training, and onboarding steps before paid support begins. Learn more about the CEN Program on Oregon.gov.
What are an ISP and ONA?
ISP: Your Individual Support Plan is your plan for I/DD services and supports. It includes what matters to you, your goals, and the help you need. ONA: If you need an Oregon Needs Assessment, it helps identify your support needs, risks, and level of care. Not everyone needs an ONA. Your Services Coordinator or Personal Agent can explain what applies to you. Learn about the ISP or learn about the ONA on Oregon.gov.
How do I start services?
Start customer intake, use the Find Your Care Team tool, or call us. A team member usually follows up within 1 to 2 business days to understand your situation and explain the next step.
Do you serve my county?
Empowered Services accepts service inquiries from all 36 Oregon counties. Every county has clear county-level care accountability within the regional model. Service start and matching timelines depend on approved services and DSP availability.
Can someone I already know become my paid caregiver?
Often, yes. A family member, friend, or longtime supporter may apply through the Empowered Choice Program. They must complete background checks, required documents and qualifications, onboarding, training, and employment steps before providing paid support.
What happens after services begin?
After services begin, your care team stays connected. Your Care Manager and Care Coordinator help with changes, backup support, required reports, questions, concerns, and connections to local resources and events.
What if my DSP cannot come one day?
We provide backup support when your regular DSP cannot work. Contact us as soon as you know a shift may be missed. We will use your backup plan, coordinate qualified coverage, and tell you who is following up. The timing of replacement coverage depends on qualified DSP availability.
How much do services cost?
For eligible people, approved I/DD services are generally funded through Oregon’s I/DD system. Your local Community Developmental Disabilities Program (CDDP) determines I/DD eligibility. Your Services Coordinator or Personal Agent helps you build and update your ISP and understand your authorized services. There is no cost to ask Empowered Services about starting services.
