Our Regional Care Model

Care organized locally — and around you.

Empowered Services is certified statewide, accepts intake statewide, and recruits caregivers across Oregon. You may bring someone you already trust or ask us to help create a match. Dedicated regional care teams are currently established in 11 counties as the model grows statewide. Your Regional Care Manager leads the region where assigned. Your Care Coordinator may work from Head Office, locally in the field, or across both functions — supporting intake, matching, onboarding, ongoing services, and follow-through. Your DSP or Empowered Choice provider delivers day-to-day attendant care at home and in the community.

The Journey

From first call to everyday life, step by step.

Different specialists contribute at different stages, but you should not have to navigate separate departments on your own. Your Regional Care Manager leads the regional model where assigned. Your Care Coordinator supports coordination throughout the journey and may work from Head Office, locally in the field, or across both functions. In some regions, the same Care Coordinator stays connected across multiple stages.

    Empowered Services is certified statewide, accepts intake statewide, and recruits caregivers across Oregon. Dedicated regional care teams are currently established in 11 counties as the model grows statewide. Your location helps us connect you to the appropriate Care Coordinator and specialists for your area, and to a Regional Care Manager where a regional team is established.

  2. Intake Specialist

    Intake & agency onboarding

    Your Intake Specialist confirms your location and current service status, gathers the information Empowered needs, and coordinates with your Service Coordinator or Personal Agent as agency onboarding and the service agreement are completed.

    Empowered Services does not determine IDD eligibility or authorize service hours. Eligibility and authorization are handled through Oregon’s IDD and case-management processes.

  3. Recruiter

    Recruiting & readiness

    A Recruiter posts customized job ads on our careers page and job boards. Before any match moves forward, we review what your plan authorizes and any personal support plans or protocols.

  4. Matching Specialist

    A thoughtful match that you approve

    Your Matching Specialist coordinates the matching process, reviews candidate profiles against the required support, and recommends candidates for a Meet & Greet. You decide whether a proposed match moves forward.

    A match may begin through agency-led recruiting, a candidate referred to Empowered, or Empowered Choice. The EmpoweredMatch App will add another discovery option in the future.

  5. DSP or Empowered Choice provider

    Care begins

    Your DSP or Empowered Choice provider does the day-to-day services as per the care support plan — meeting goals and working toward the outcomes that matter to you, as authorized in your plan.

  6. Care Coordinator

    Your Care Coordinator supports ongoing services

    Your Care Coordinator supports communication, monthly check-ins, quarterly reporting, service changes, backup planning, and next steps with your Service Coordinator or Personal Agent — and connects you with events and resources in your community. Depending on the region, your Care Coordinator may work from Head Office, locally in the field, or across both functions.

    In some cases, the same Care Coordinator supports multiple stages of your journey. In others, responsibilities may transition within Empowered while your documented preferences, current priorities, and agreed next steps remain connected. Your Regional Care Manager remains available for regional leadership, support, and escalation where assigned.

  7. Regional Care Manager

    Regional leadership stays accountable

    Where the regional model is established, your Regional Care Manager leads the region’s Care Coordinators, DSPs, and Empowered Choice providers — accountable for quality of care, customer and family experience, workforce support, community partnerships, compliance, and escalation.

Care Coordinator

Supports coordination across intake, matching, onboarding, and ongoing services. Depending on the region, may work from Head Office, locally in the field, or across both functions. Coordinates check-ins, reporting, communication, documentation, service changes, scheduling, backup planning, and follow-through.

DSP or Empowered Choice provider

Your caregiver. Delivers authorized day-to-day support — at home and in the community — working toward your goals and outcomes.

Regional Care Manager

Leads the regional team from day one. Accountable for quality of care, customer and family experience, workforce support, local partnerships, compliance, and escalated concerns.

Care Coordinator is the official title. The regional model is being phased in and expanded, so one Care Coordinator may carry Head Office and field responsibilities or support more than one stage of the journey. We tell you who is assigned, what they own, and how to reach them.

After Services Begin

A rhythm you can count on.

Ongoing coordination follows a defined rhythm — not only when something goes wrong. Monthly check-ins, quarterly reviews, written reporting, and documented follow-through help the team spot changes early and keep next steps clear.

Monthly

Separate customer & DSP check-ins

We check in with you — and, when you choose or it is legally authorized, your representative or supporter — and separately with your DSP. Each person has space to share what is working, what is changing, and what follow-up is needed.

Quarterly

Relationship & progress review

A structured review of how the support relationship is working, progress toward identified goals, and any changes that may be needed.

For case-management partners

Written quarterly progress report

We send your Service Coordinator or Personal Agent a written quarterly progress report.

Ongoing

Follow-through with a clear owner

Your Care Coordinator coordinates scheduling, backup planning, documentation support, and changes in support needs — and helps identify barriers, communicate concerns, and coordinate next steps with your Service Coordinator, Personal Agent, or other chosen support-team members.

Locally connected: regional resources, community events, newsletters, and partnerships help customers, families, and DSPs stay connected beyond scheduled services.

How we support continuity

Things change — that is normal. What never changes: someone owns it, your story is kept, and you know what happens next.

You approve the match

A Meet & Greet is arranged for every match — except Empowered Choice, where you are bringing someone you already know. Services move forward only with your agreement.

Backup preferences are planned

We document what should happen when the regular DSP is unavailable and work to introduce backup options when possible.

Changes have a clear owner

Your Care Coordinator tells you who is coordinating a missed shift or service concern and what happens next.

Your history stays connected

If a DSP or coordinator changes, Empowered maintains the documented preferences, current priorities, open action items, service information, and agreed next steps — so you do not have to start over.
