Empowered Services is certified to provide the following Oregon I/DD services. The person’s approved plan and authorization guide what may be appropriate; availability also depends on age, location, qualified staffing, schedule, and agency capacity.

What I/DD services does Empowered Services offer in Polk County?
Empowered Services is certified to provide authorized attendant care, relief care, behavior support plan implementation, on-the-job attendant care, day support activities, and community transportation. What is available for a person depends on the ISP, authorization, age, location, qualified staffing, schedule, fit, and agency capacity.
Who decides I/DD eligibility?
Polk County Developmental Disabilities determines eligibility. Call the county at 503-831-5969. Empowered Services can explain its service process but does not decide eligibility or authorize service hours.
Can I bring a caregiver I already trust?
Often, yes. A family member, friend, or other trusted person may be considered for employment as the paid DSP when services are authorized and the person meets hiring, background-check, and training requirements. This pathway is called Empowered Choice.
Who is on the Polk County care team?
