Empowered Services is certified to provide the following Oregon I/DD services. The person’s approved plan and authorization guide what may be appropriate; availability also depends on age, location, qualified staffing, schedule, and agency capacity.
Attendant Care
Support with personal care, meals, mobility, safety, communication, and daily routines.
Relief Care
Trusted support when the usual caregiver needs a break or cannot be there.

What I/DD services does Empowered Services offer in Linn County?
Empowered Services is certified to provide authorized attendant care, relief care, behavior support plan implementation, on-the-job attendant care, day support activities, and community transportation. What is available for a person depends on the ISP, authorization, age, location, qualified staffing, schedule, fit, and agency capacity.
Who decides I/DD eligibility?
Linn County Developmental Disabilities determines eligibility. Call the county at 541-967-3890. Empowered Services can explain its service process but does not decide eligibility or authorize service hours.
Can I bring a caregiver I already trust?
Often, yes. A family member, friend, or other trusted person may be considered for employment as the paid DSP when services are authorized and the person meets hiring, background-check, and training requirements. This pathway is called Empowered Choice.
Who is on the Linn County care team?
