What I/DD services does Empowered Services offer in Clackamas County?
Empowered Services is certified to provide authorized attendant care, relief care, behavior support plan implementation, on-the-job attendant care, day support activities, and community transportation. What is available for a person depends on the ISP, authorization, age, location, qualified staffing, schedule, fit, and agency capacity.
Who decides I/DD eligibility?
The Clackamas County Community Developmental Disabilities Program determines eligibility. Call the county intake and eligibility line at 503-557-2824. Empowered Services can explain its service process but does not decide eligibility or authorize service hours.
Can I bring a caregiver I already trust?
Often, yes. A family member, friend, or other trusted person may be considered for employment as the paid DSP when services are authorized and the person meets hiring, background-check, and training requirements. This pathway is called Empowered Choice.
Who is on the Clackamas County care team?
