If a family member, friend, neighbor, or current customer has chosen you as their preferred caregiver, the Customer Care Intake form helps us document the customer's support needs and their choice of you as caregiver.
You do not need to submit another DSP interest form. But the customer, family, case manager, or you together can complete the Customer Care Intake to begin the Empowered Choice Program pathway.
